Job description
The Interface team was set up to allow patients who require palliative care to return home and receive care in the best possible conditions. It travels to their homes but also to a nursing home, a rest and care home or another place of accommodation.
She works with the general practitioner and the caregivers who provide care on a daily basis (1st line team), in a spirit of consultancy, partnership and continuity. It offers help in the form of information, advice and support and ensures a link with the Cliniques universitaires Saint-Luc.
The Interface team offers, in accordance with the wishes and needs of each person:
Listening and support for the patient and their loved ones
An aid to relieve pain and other symptoms.
Advice and support to improve well-being and provide care in the best possible conditions.
The search for nursing staff: nurses, family helpers, physiotherapists, nurses, psychologists, volunteers, etc.
The search for specialized care equipment.
Assistance with certain social procedures.
Support for the 1st line team (with a 24-hour service).
Profile
Qualifications and experience required:
You have a bachelor's degree in nursing
You hold the qualification in palliative care
You have experience in hospital or home care as well as experience working with patients at the end of life
You have a B driving licence
Essential skills :
You have a perfect knowledge of French (level B2)
You have listening skills and communication skills
You have a good control of your emotions
You are able to take responsibility
You have a sense of organization and ensure continuity of care
You like to work in a multidisciplinary team
You demonstrate the ability to observe, discern and judge
You are flexible and available
You have the ability to think ethically
You are concerned about your professional development and the evolution of the nursing profession
You are comfortable with computer tools
Additional assets:
You have a specialization in oncology or training in algology, ethics, systems analysis or another discipline that can contribute to improving the quality of services
You have a basic knowledge of Dutch and English
